BC Student Information Verification Forms
Every year, we ask you to review, update, and return our BC Student Information Verification Form – this form has been sent home with your child today.
This process helps us keep our records up to date and is a requirement of the B.C. Ministry of Education and Child Care.
Please look over the information printed on the form and return the form to the office if there are any changes required. It is important that any returned form with changes also
includes a parent or guardian signature. Please open the attached letter with more detailed information about the form.
You can write changes directly on the form and return it to the school with your child by Friday, September 18th
